Heather Pisano, MS, APRN, FNP-C, PMHNP-BC is a dual board-certified nurse practitioner holding certifications in both Family Health (FNP-C) and Psychiatric Mental Health (PMHNP-BC). She has over ten years of clinical experience spanning internal medicine, hematology-oncology, and hospice and palliative care, experiences that shaped her ability to manage medically complex patients with both precision and compassion. This foundation allows her to approach mental health and hormone care with a deep understanding of the interconnected nature of physical and psychological well-being.

Heather has completed advanced training in women’s mental health, integrative psychiatry, and hormone-informed psychiatric care, including training through Heather Hirsch Academy and Kate Salama, MD’s Tend to Her HRT for Mental Health. This education supports her evaluation of hormonal influences on women’s mental health, particularly PMDD, perimenopause, and menopause. When clinically indicated, she considers hormone therapy alongside evidence-based psychiatric treatment.

Her psychiatric practice focuses on anxiety disorders, OCD and intrusive thoughts, mood disorders including depression and bipolar spectrum conditions, ADHD in women, PTSD and complex PTSD, and the intersection of hormonal transitions and mental health, including PMDD and perimenopausal mood changes. She combines careful medication management with an integrative lens.

Heather founded hp | np Health to create a practice model that prioritizes time, access, and continuity. After years in high-volume clinical environments, she wanted to build a setting where appointments allow for meaningful discussion, thorough evaluation, and individualized plans of care. Her model emphasizes longer visits, direct access, and care decisions guided by clinical judgment rather than insurance constraints.

In-person care is available in Elma, NY, serving Buffalo and Western New York. Secure telehealth services are available to patients located in New York. She is committed to providing thoughtful, integrative care to women throughout Western New York and beyond.

“Evidence-Based Care practiced with patience and genuine presence.”

A Philosophy of Care

The Whole Person

Mental, hormonal, and metabolic health are deeply interconnected. Every patient is approached as a whole person, recognizing that what happens in the body affects the mind, and vice versa. This integrated perspective guides every aspect of care.

Root Causes, Not Just Symptoms

Symptoms are meaningful signals. Rather than simply suppressing them, the goal is to understand what may be contributing to them, whether hormonal changes, sleep disruption, inflammation, or life circumstances that deserve attention. Addressing these underlying factors allows for more durable and meaningful change.

Evidence-Based & Integrative

Evidence-based medicine is the foundation, while recognizing that effective treatment often extends beyond medication alone. Lifestyle medicine, targeted supplementation, and mind-body strategies all have a growing research base and are incorporated thoughtfully into care.

Time & Attention as Medicine

The therapeutic relationship itself is healing. This practice was built on the belief that unhurried appointments, thoughtful listening, and continuity of care form the foundation of good medicine.

Collaboration & Transparency

Patients are partners in their own care. Clinical reasoning is explained, options are discussed rather than dictated, and questions and preferences are welcomed into every decision. Informed, collaborative care produces better outcomes.

Compassion Without Judgment

Whether navigating a mental health challenge, a hormonal transition, or struggles with weight, every patient deserves care that is free from shame and judgment. Each person is approached with warmth, respect, and the understanding that health is complex and deeply personal.
